THE PROBLEM OF MOTIVATION AND PAYMENT OF MEDICAL STAFF

Abstract. In the article mechanisms of motivation and system of remuneration of health workers are considered. The urgency of the problem of labor motivation in health care is extremely acute. In the Russian public health services, retrospective methods of payment are predominantly used, which lead to a decrease in the efficiency of the use of labor and material resources. Understanding the processes of labor motivation, it is possible to develop a more productive system of influence on the employees by the manager.

Nowadays our state pays a great attention to public health service. Every year quantity of illnesses increases, sometimes scientists can’t find the solution to this problem. Though some improvement of health state of population has been achieved for ten last years in RF, we fall behind the leading countries. These are eminently qualified experts who can develop health protection. Only owing to them quality of health service can be high, the result of it, population health will increase. According to investigations people want to be treated by experienced doctors who have academic degrees. This fact is not depended on people’s living standards. It means as many as possible qualified experts must be interested and recruited by our medical establishments in order to increase people attendance factor. Medical staff is totality of individuals who have their own needs, interests, faculties. Two conceptions must be distinguished: motivation and stimulation.
Stimulation is an outside impact on the result of an employee’s work granting him/her bonuses, fringe benefits remunerations and so on. Applying them the head physician is only able to motivate his/her employees for effective work.
The solution of the problem of increasing motivation for effective work is the most important duty of the health service administration. Motivation can be represented as the process of impact on a person in order to induce him/her to definite actions. Medical employees have appointed several factors which influence upon their efficiency.
They are financial stimulus opportunities for professional development and opportunities for making his/her career, good relations in the collective. The most important element is financial one. If salary corresponded to the volume of accomplished work their such factor as improvement of labour conditions would be significant. The value statement of the third side (a patient) is one of a clear motivation stimuli are divided into external internal ones. Trust of the patient is an example of external stimuli, the oath of the Hippocrates is an example of internal ones. The management of a medical establishment should carry out investigations among medical staff periodically in order to find individual approach to this or that employee. Many countries have the following payment system: a medical employee receives, a fixed salary, but he/she gets a fee for overtime work. There are two kinds of payment: a fixed salary is made for caring out some services (a piece — rate form of payment), a fee is paid for additional hour of work (time — based form of payment). I.e amount of fee depends on the employee’s decision directly. This payment system is good for an employer as well, since the overall productivity of labor rises. There is salary payment, which depends on the number of patients. In this case a specialist can examine healthier people because it won’t take much time and efforts to treat them. Which the same goal the doctor won’t allow further deterioration of the patients’ state of health. In general, these two systems have their own advantages and disadvantages, that is why there is a tendency to mix them. The state is trying to help medical staff financially. In 2013 innovation of labour remuneration were presented. The payroll of a medical institution may be changed depending on the number of patients. The more people are treated, the more money is allocated to them, thus the salary of employees increases. The following pattern can be singled out: labor productivity of employees — evaluation and response of patients — employee salary. In conclusion, I want to say that we should not forget about the law of diminishing marginal productivity so material support without reason serves as the factor when increasing, the time is sure to come when productivity of labour will decline.

Аннотация. В статье рассматриваются механизмы мотивации и системы оплаты труда работников сферы здравоохранения. Актуальность проблемы мотивации труда в сфере здравоохранения чрезвычайно остра. В Российском здравоохранении преимущественно используют ретроспективные методы оплаты труда, которые приводят к снижению эффективности использования трудовых и материальных ресурсов. Понимая процессы мотивации труда, можно выработать более продуктивную систему воздействия на работников со стороны руководителя.
